Team of doctors at Sahyadri Hospital perform a complex procedure in a young patient suffering from complex blood disorder

Pune (P&V News Service) : A team of doctors at Sahyadri Super Specialty Hospital Hadapsar successfully treated a young patient who had landed up in a complex situation due to Immune thrombocytopenic purpura blood disorder.

Explaining the case , Dr. Chandrakant Lahane , Consultant Haematologist Haemato-Oncologist BMT Physician at Sahyadri Super Specialty Hospital Hadapsar said that a 29 year women suffering from Immune thrombocytopenic purpura ( ITP ) was admitted to the with complaints of abdominal pain and dizziness. Preliminary examination of this patient showed that her blood pressure was very low and blood test showed that her hemoglobin had reduced to four  and platelet  were only 9000 (normal count of platelets 1,50,000 -4.5,00000).

What is ITP ?

Immune thrombocytopenic purpura (ITP) is a blood disorder characterized by a decrease in the number of platelets in the blood. Platelets are cells in the blood that help stop bleeding. A decrease in platelets can cause easy bruising, bleeding gums, and internal bleeding. This is an autoimmune condition in which antibodies target blood platelets.

Sonography revealed that the patient has an additional hemorrhage in the abdomen and a mass near the patient’s ovary that could have been  the source of the bleeding said Dr Lahane . In effect two bags of blood and six bags of platelets were given to the patient.

A team of doctors including Dr. Chandrakant Lahane, General Surgeon Dr. Phulchand Pujari, Gyanacologist  Dr. Manisha Kulkarni and Anaesthetist Dr. Aparna Kaje chalked out the future course of action and discussed this with the relatives of the patient.

Dr Lahane said that the it was necessary to find the source of bleeding and surgically remove the spleen (splenectomy)  to stop the destruction of her platelets. The challenge was the minuscule count of platelets and significantly reduced level of hemoglobin. After taking the relatives into confidence our team went ahead and performed the surgery which was highly risky and complex in these circumstances. This was an open surgery as we had to find the source of bleeding . We found that the bleeding was happening due to ovulation . Normally this does not happen, but in this case it was due to low platelet count.

We are satisfied that post the surgery , the patient recovered well and returned home safely on the 12th day with 50,000 /cmm platelet count . She will have to take medications for a few years , immunizing vaccines for Pnue moccooal , H influenza , Meningococcal. The good thing is that with proper precautions she can lead a normal life said Dr. Lahane .
