Over the last six months, she began experiencing worsening symptoms, including abdominal pain, heavy menstrual bleeding, and urinary discomfort, which gradually disrupted her daily routine and quality of life. Although she had earlier avoided surgery due to fear of an open procedure and prolonged recovery, the increasing severity of symptoms eventually led her to seek surgical treatment.

Speaking about the case, Dr. Kapil Kanade, HOD & Consultant – Obstetrics & Gynaecology, Manipal Hospital Kharadi, said, “After the patient presented at the hospital and underwent detailed medical tests, multiple uterine fibroids—subserosal, intramural, and submucosal—were identified, which had enlarged the uterus to the size of a 28–30-week pregnancy. The largest fibroid measured approximately 18 × 17 cm, with the combined weight of the uterus and fibroid around 1.8 kg. Such advanced presentations are increasingly being seen today, largely due to delayed diagnosis and hesitation towards surgical intervention. This was a high-risk because of the massive uterine size, previous caesarean sections leading to dense internal adhesions, and the increased risk of heavy bleeding and bladder injury.”

Further he added, “Despite these challenges, a total laparoscopic hysterectomy was successfully performed using advanced minimally invasive techniques. The uterus and fibroids were removed through three small incisions measuring 5–8 mm, with minimal blood loss of around 200 ml. The patient recovered well and benefited from faster recovery and minimal scarring.”

Mr. Parameswar Das, Hospital Director, Manipal Hospital, said “Managing such complex cases through minimally invasive surgery reflects our strong clinical expertise and advanced infrastructure. At Manipal Hospital, we remain committed to patient-centric care by offering cutting-edge surgical techniques that ensure safety, quicker recovery, and better outcomes”

The patient was discharged within 24 hours of the procedure and returned to Mumbai after a follow-up visit. Doctors advise that symptoms such as heavy menstrual bleeding, persistent abdominal pain, and changes in bowel or bladder habits should not be ignored and require timely medical consultation. Early detection not only reduces surgical risk but also helps preserve quality of life.

The woman, a mother of two with both deliveries conducted via caesarean section, had been diagnosed with uterine fibroids two years ago.
